1)  Q If a student needs to repeat 6th grade, should he/she receive a Tdap vaccination?

Yes. Any student 11y. and older can and should receive the Tdap vaccination.
2)  Q What forms are needed for a student to be able to take medication at school?
A
It is best if a student is given medication at home by a parent or guardian. However some medication must be given at school to ensure proper timing. Parents/guardians must fill out a Medication and Release Form in order for a school employee to administer the medication. In addition, a doctor's order or a prescription is needed for a student to be able to take medication at school. Appropriate forms available under the Documents Tab.
