From the Counseling Office:

When coming to MCHS, new enrollees need to bring the following items with them:
1.   Proof of Residency - Current water, gas or electric bill from July or August 2010 (Cut-Off notices can not be accepted)
2.   Copy of student's current transcript from previous school and other class placement paperwork (i.e. - IEP)
3.   Copy of shot records for your student.
Acceptable Proof of Residences are:
1.   Current gas, electric, or water bill in parent/guardian name.  Cut-off notices are not acceptable!
2.   Lease agreement/house contract reflecting property, address, phone number will be acceptable if all bills are paid by the leaser.  Other verification may be required.
3.   Resident Affidavit - one family living with another family.  This must be obtained from Central Enrollment.
